Please bring two of the following documents to show proof of residency: a current Illinois driver’s license, or photo ID voter’s registration card, current lease contract, a banking check with current address on it, utility bill stubs. Parents must accompany children who apply for a card and will need to submit address documentation and sign a statement of responsibility for library materials.
